High Volume Injection (HVI)

A high-volume injection (HVI), also known as hydrodilatation, is a highly effective treatment for adhesive capsulitis (frozen shoulder). This procedure helps to expand the constricted joint capsule, which is responsible for limiting movement.

How It Works

The HVI can lead to fast improvement in frozen shoulder mobility, particularly in cases with severe restrictions. This shot normally contains a combination of the following:

  • Steroid – to reduce inflammation
  • Local anaesthetic – to provide immediate pain relief
  • Water or normal saline – to help stretch and expand the joint capsule

Procedure Details

A local anaesthetic is first delivered into the soft tissues from the skin to the joint, entering from the back of the shoulder. The second needle is inserted into the shoulder joint with the guidance of an ultrasound after a few minutes. The steroid, anaesthetic, and saline mixture is then administered using a 3-way tap. The ultrasound ensures accuracy, allowing the fluid to expand the joint capsule effectively.

The entire process takes less than five minutes. Some patients may experience a temporary “dead arm” feeling. But, many notice immediate progress in movement, even while getting dressed after the procedure.

Further pain relief and mobility improvements typically continue over the next 4 to 6 weeks.

Other Uses of High-Volume Injections

In addition to frozen shoulder, HVI is also helpful in treating conditions like Achilles and Patellar tendinopathy. In these cases, the solution is injected in order to isolate the deep portion of the tendon from the fat pad to effectively reduce pain and sensitivity.
